-Childs name & Age:
-Height/Weight/Teeth:
-4 categories to be highlighted on sides:
-1 category to be highlighted on bottom:
-12-16 likes/loves:
*Categories can be anything you'd like, answers just must be equivalent to samples shown or custom fee needs to be added to cart. 

To change colors, add/alter graphics, add over above limitations select custom when adding to cart.  Select double custom for more than one of these changes.
